In one prospective study cited by the EAU, men used a traction device for at least four hours per day for six months and achieved average gains of about 2.3 cm in flaccid length and 1.7 cm in stretched length.
Another study cited by the guideline reported gains after several months of daily use, with average increases of roughly 1 to 1.7 cm depending on how length was measured.
The signal is real enough to take seriously, but not strong enough to promise a specific result. The studies are small, protocols vary, adherence matters enormously and the evidence is not comparable to a large, high-quality drug trial.
Mayo Clinic takes a similarly cautious position: a few small studies have reported length increases of roughly half an inch to almost two inches, or about 1 to 3 cm, but more research is needed to confirm safety and effectiveness.
How Much Length Can a Penis Extender Add?
The realistic answer is usually modest change, not dramatic transformation.

This is one of the more medically established uses of penile traction.
European guidance notes that traction devices may help reduce penile curvature and recover some lost length in men with Peyronie's disease, although the studies remain limited and heterogeneous.
Meta-analyses cited by the EAU suggest traction can improve curvature and may increase stretched penile length, particularly when used as part of a wider treatment plan.
If you have Peyronie's disease, that is a medical-treatment conversation rather than a cosmetic-enlargement one. Device choice and protocol should be discussed with a urologist.
Are Penis Extenders Safe?
Serious complications appear uncommon in published traction studies, but discomfort and inconvenience are common enough to matter.
