Penis Enlargement: What Actually Works and What Is Mostly Marketing

Penis enlargement is one of those markets where “before and after” photographs are much easier to find than good evidence.

Some penis-enlargement methods can change size. That is the part the internet often gets technically right.

What it leaves out is how much change is realistic, whether the change is permanent, how long treatment takes, what complications can occur and whether the treatment actually improves sex or confidence afterwards.

The evidence is not “nothing works.” It is much narrower: traction devices can produce modest length gains in some men; medically performed fillers can increase girth; certain operations can alter visible length or circumference. Pills, creams, jelqing and ordinary vacuum pumping have not been shown to permanently enlarge a normal adult penis.

Before Enlargement: What Is an Average Penis Size?

A widely cited systematic review pooled measurements taken by health professionals from up to 15,521 men.

The average measurements were approximately:

  • erect length: 13.12 cm,
  • stretched flaccid length: 13.24 cm,
  • flaccid length: 9.16 cm,
  • erect circumference: 11.66 cm.

Those numbers are averages, not minimum requirements for satisfying sex.

There is also substantial natural variation.

Because visual impressions can be distorted by:

Penile Traction Devices: The Best-Supported Non-Surgical Option

The EAU guideline currently considers traction a conservative option for increasing penile length.

Small prospective studies have reported average gains around:

  • roughly 1–2 cm in stretched or erect length in some cohorts,
  • sometimes more in flaccid length,
  • little or no meaningful increase in girth.

Older studies often required several hours of daily wear for months.

That is a serious adherence burden.

The EAU concludes that traction appears effective for lengthening, but explicitly notes that the evidence quality is poor because available studies are small, heterogeneous and often lack strong randomized controls.

wear it for ten minutes and gain three inches.

Does Penile Traction Increase Girth?

Not reliably.

The EAU specifically notes that penile traction appears useful for lengthening but is not effective for girth enhancement.

If a traction-device advertisement promises dramatic permanent increases in both length and circumference, that claim goes beyond the evidence.

Do Penis Pumps Permanently Increase Size?

There is no good evidence that ordinary vacuum erection devices permanently enlarge a normal penis.

A vacuum pump pulls blood into the penis.

That can make it:

  • firmer,
  • more engorged,
  • temporarily larger.

That is why pumps can be medically useful for erectile dysfunction.

It is not the same as permanent tissue enlargement.

Mayo Clinic states that pumps have not been proved to work for penis enlargement, and the EAU cites a study in which six months of vacuum-device use failed to produce significant length increases.

No oral supplement has been shown to permanently enlarge a normal adult penis.

  • amino acids,

  • ingredients marketed as testosterone boosters.

None has good evidence for permanently increasing penile length or girth.

some “male enhancement” supplements contain undeclared pharmaceutical ingredients.

The FDA maintains an active list of sexual-enhancement products found to contain hidden substances.

In 2026 alone, FDA alerts have included products containing undeclared tadalafil or dangerously high amounts of yohimbine.

“Natural male enhancement” does not automatically mean safe.

If an unregulated supplement appears to work suspiciously like prescription erectile-dysfunction medication, there may be a reason.

What About Creams, Oils and Enlargement Gels?

There is no established topical cream or oil that permanently increases adult penile size.

Temporary warmth, increased blood flow or skin swelling should not be confused with tissue enlargement.

Anything claiming permanent centimetres from rubbing a substance onto the penis should be treated skeptically.

Does Jelqing Work?

Jelqing is a manual stretching or milking exercise in which pressure is repeatedly moved along the penis.

It is heavily promoted online.

Good scientific evidence showing reliable permanent enlargement is lacking.

Mayo Clinic notes that there is no scientific proof that jelqing works and warns that it may cause:

  • pain,
  • scar formation,
  • disfigurement.

Both involve mechanical forces.

Do not assume that more force means faster growth.

Uncontrolled loading can injure penile tissue, nerves and blood vessels.

Can Penile Fillers Increase Girth?

Yes.

This is one of the areas where the answer is genuinely different from the supplement market.

Injectable soft-tissue fillers can increase penile circumference.

The EAU reports studies of hyaluronic-acid filler showing average girth increases in the range of roughly 1.4 to 3.78 cm.

However:

  • the evidence is still limited,
  • follow-up is not always long-term,
  • results depend heavily on practitioner skill,
  • temporary swelling and pain can occur,
  • filler may require maintenance or revision.

Important guideline nuance

The EAU gives only a weak recommendation for hyaluronic acid, selected soft-tissue fillers and autologous fat injection for girth enhancement. It strongly advises extensive counselling and strongly recommends against injecting silicone, paraffin or petroleum jelly because severe complications can occur.

Hyaluronic Acid Is Not the Same as Injecting Silicone or Oil

These procedures are sometimes discussed online as though “penis injections” were one category.

They are not.

Medical hyaluronic-acid filler administered by an appropriately trained clinician is fundamentally different from injecting:

  • industrial silicone,
  • liquid silicone from an unregulated source,
  • paraffin,
  • mineral oil,
  • petroleum jelly.

The EAU warns that these substances can cause complications ranging from inflammation and infection to severe tissue damage requiring reconstructive surgery.

Do not inject substances into the penis yourself.

Does Fat Transfer Increase Penis Girth?

Autologous fat transfer takes fat from another part of the body and injects it around the penile shaft.

Published studies have reported increases in circumference.

But there are unresolved issues:

  • some fat can be reabsorbed,
  • results may be uneven,
  • nodules or contour irregularities can occur,
  • long-term evidence is limited.

Professional guidance is not completely aligned here.

EAU: allows autologous fat injection as a weak recommendation after counselling.

AUA: states that subcutaneous fat injection for increasing penile girth has not been shown to be safe or effective.

That disagreement is a good reason not to treat fat transfer as an established routine cosmetic procedure.

Does Penis Lengthening Surgery Work?

Some operations can increase visible penile length.

The best-known cosmetic procedure is suspensory ligament release.

The suspensory ligament connects the penis to the pubic area. Cutting it can allow more of the internal shaft to hang externally.

But there are limitations.

  • The visible gain may be greater when flaccid than erect.
  • The angle of erection can change.
  • Scarring and other surgical complications are possible.
  • Patient satisfaction is not guaranteed.

What About Removing Suprapubic Fat?

This is different from making penile tissue longer.

In men with significant suprapubic fat or an adult acquired buried penis, reducing tissue around the pubic area can expose more of the existing shaft.

The penis may therefore appear longer without the erectile tissue itself having been enlarged.

For medically significant buried penis, surgery may also improve hygiene, urination and sexual function.

That is a different clinical situation from cosmetic enlargement of a normal exposed penis.

Does Testosterone Make the Penis Bigger?

Not in a normal adult after puberty.

Hormonal treatment can be medically important in boys with micropenis or certain developmental/endocrine conditions.

Potential complications across invasive enlargement techniques include:

  • pain,
  • swelling,
  • infection,
  • scarring,
  • nodules or irregular contours,
  • filler migration,
  • changes in sensation,
  • erection problems,
  • dissatisfaction with the cosmetic result,
  • need for corrective surgery.

Does Enlargement Improve Sexual Satisfaction?

Not automatically.

Increasing a measurement does not guarantee:

  • better erections,
  • better orgasm,
  • a more satisfied partner,
  • greater relationship security,
  • lasting confidence.

The EAU notes that cosmetic treatments may improve genital self-image or self-esteem for some people, but outcomes for sexual function, relationship satisfaction and psychological adjustment are mixed.

The distress can be very real.

  • What complications have you personally treated?
  • What happens if I dislike the result?
  • Can the treatment affect erection angle, sensation or erectile function?
  • What professional guideline supports this procedure?

If the clinic cannot answer those questions without switching immediately into sales language, that is useful information too.

The Bottom Line

Penis enlargement is not completely fictional.

But the evidence is much less exciting than the advertisements.

Traction: modest length gains are possible with months of consistent use.

Pumps: useful for erections, not proven permanent enlargement.

Pills and creams: no good evidence for permanent size increase.
